Making your own icon packages
You
don't have to rely on icon packages made by others either, creating your own icon
packages is very easy. You can click on
the
"Icons and Cursors" toolbar item and drag and drop the replacement icons right in
there.
Or you can go to theView->Package Builder which will bring up a sheet of every icon type on your system. From there, simply drag and drop your own icons on there to create your own package. Once done, you can save your package for later use.
Or do you mean how do you add ico and formats?

In settings, uncheck the "Save package as individual icons" in the Saving section
Then, File | Save Package As...
Give it a name and save. Check out teh folder that was created.
Sorry about that other version.
Where is the settings menu in IconPackager
Obviously should be in File or Tools but is in none of the menus...
